In this open peer commentary, we categorize the possible “neuroscience in national security” definitions of misuse of science and identify which, if any, are uniquely presented by advances in neuroscience. To define misuse, we first define what we would consider appropriate use: the application of reasonably safe and effective technology, based on valid and reliable scientific research, to serve a legitimate end. This definition presents distinct opportunities for assessing misuse: misuse is the application of invalid or unreliable science, or is (...) the use of reliable scientific methods to serve illegitimate ends. Ultimately, we conclude that while national security is often a politicized issue, assessing the state of scientific progress should not be. (shrink)

Software application ontologies have the potential to become the keystone in state-of-the-art information management techniques. It is expected that these ontologies will support the sort of reasoning power required to navigate large and complex terminologies correctly and efficiently. Yet, there is one problem in particular that continues to stand in our way. As these terminological structures increase in size and complexity, and the drive to integrate them inevitably swells, it is clear that the level of consistency required for such navigation (...) will become correspondingly difficult to maintain. While descriptive semantic representations are certainly a necessary component to any adequate ontology-based system, so long as ontology engineers rely solely on semantic information, without a sound ontological theory informing their modeling decisions, this goal will surely remain out of reach. In this paper we describe how Language and Computing nv (L&C), along with The Institute for Formal Ontology and Medical Information Sciences (IFOMIS), are working towards developing and implementing just such a theory, combining the open software architecture of L&C’s LinkSuiteTM with the philosophical rigor of IFOMIS’s Basic Formal Ontology. In this way we aim to move beyond the more or less simple controlled vocabularies that have dominated the industry to date. (shrink)

Social scientists have paid insufficient attention to the role of law in constituting the economic institutions of capitalism. Part of this neglect emanates from inadequate conceptions of the nature of law itself. Spontaneous conceptions of law and property rights that downplay the role of the state are criticized here, because they typically assume relatively small numbers of agents and underplay the complexity and uncertainty in developed capitalist systems. In developed capitalist economies, law is sustained through interaction between private agents, courts (...) and the legislative apparatus. Law is also a key institution for overcoming contracting uncertainties. It is furthermore a part of the power structure of society, and a major means by which power is exercised. This argument is illustrated by considering institutions such as property and the firm. Complex systems of law have played a crucial role in capitalist development and are also vital for developing economies. (shrink)

The central hypothesis of the collaboration between Language and Computing (L&C) and the Institute for Formal Ontology and Medical Information Science (IFOMIS) is that the methodology and conceptual rigor of a philosophically inspired formal ontology greatly benefits application ontologies. To this end r®, L&C’s ontology, which is designed to integrate and reason across various external databases simultaneously, has been submitted to the conceptual demands of IFOMIS’s Basic Formal Ontology (BFO). With this project we aim to move beyond the level of (...) controlled vocabularies to yield an ontology with the ability to support reasoning applications. Our general procedure has been the implementation of a meta-ontological definition space in which the definitions of all the concepts and relations in LinKBase® are standardized in a framework of first-order logic. In this paper we describe how this standardization has already led to an improvement in the LinKBase® structure that allows for a greater degree of internal coherence than ever before possible. We then show the use of this philosophical standardization for the purpose of mapping external databases to one another, using LinKBase® as translation hub, with a greater degree of success than possible hitherto. We demonstrate how this offers a genuine advance over other application ontologies that have not submitted themselves to the demands of philosophical scrutiny. LinKBase® is one of the world’s largest applications-oriented medical domain ontologies, and BFO is one of the world’s first philosophically driven reference ontologies. The collaboration of the two thus initiates a new phase in the quest to solve the so-called “Tower of Babel”. (shrink)

Social norms – rules governing which behaviours are deemed appropriate or inappropriate within a given community – are typically taken to be uniquely human. Recently, this position has been challenged by a number of philosophers, cognitive scientists, and ethologists, who have suggested that social norms may also be found in certain non-human animal communities. Such claims have elicited considerable scepticism from norm cognition researchers, who doubt that any non-human animals possess the psychological capacities necessary for normative cognition. However, there is (...) little agreement among these researchers about what these psychological prerequisites are. This makes empirical study of animal social norms difficult, since it is not clear what we are looking for and thus what should count as behavioural evidence for the presence (or absence) of social norms in animals. To break this impasse, we offer an approach that moves beyond contested psychological criteria for social norms. This approach is inspired by the animal culture research program, which has made a similar shift away from heavily psychological definitions of ‘culture’ and to become organized around a cluster of more empirically tractable concepts of culture. Here, we propose an analogous set of constructs built around the core notion of a normative regularity, which we define as a socially maintained pattern of behavioural conformity within a community. We suggest methods for studying potential normative regularities in wild and captive primates. We also discuss the broader scientific and philosophical implications of this research program with respect to questions of human uniqueness, animal welfare and conservation. (shrink)

The term empathy (Einfühlung) is rooted in philosophical aesthetics. It was used by German philosophers toward the end of the nineteenth century to describe our ability to imaginatively “feel into” works of art, which speak to us in a certain humanlike way insofar as they contain traces of what Mikel Dufrenne calls a “quasi-subjectivity” (1973, 393). In this chapter, rather than looking to art as an object of empathy, we instead consider art—and more specifically, music—as a resource that can facilitate (...) empathy. More precisely, we turn to two cases in which music seems to establish spaces that enable and sustain empathic connectedness, as well as the ability to explore and experiment with different forms of social understanding and affective sharing. The first case comes from the music tradition of free improvisation. Based on ethnographic fieldwork with the saxophone player, Torben Snekkestad, we become acquainted with an approach to performance that does not concern music primarily as an aesthetic product, but as a shared process of communicating and connecting nonverbally with others—a process that is essentially about exploring and experimenting with different forms of intersubjectivity and empathy. The second case comes from music therapy and autism, which can involve listening, singing, or joint music-making. We discuss studies indicating that musical interventions positively address core impair- ments in capacities required for empathy: for example, joint attention, social reciprocity, and verbal and nonverbal communication, as well as comorbidities of atypical perception, motor performance, and behavioral problems. Although these two cases are different, they share the trait of using music to establish spaces of nonverbal communication and empathic understanding. Drawing on these two cases, this chapter analyses how musical-empathic spaces emerge and how these spaces solicit the integration of both low-level affective and bodily resonance mechanisms as well as high-level acts of meta-reflection, imagination, and planning that contribute to different forms of empathic understanding. The three young philosophers Kevin Mulligan, Peter Simons, and Barry Smith have become well known in just a few years, especially among German-speaking analytic and phenomenological philosophers. This is due not only to their historical and systematic philosophical work, but also to their provocative way of advocating for their philosophy. Because they often appear as a trio, they are sometimes called the “Gang of Three” or—admiringly—”Mafiosi” (as Nicholas Rescher once put it).

This article makes six points. First, under any plausible normative perspective, the distinction between mistake (and ignorance) of criminal law and mistake of fact must at least sometimes be drawn. Second, the fundamental distinction is between a mistake about the state’s authoritative statement of what is prohibited (“M Law”), and a mistake about whether that prohibitory norm is instantiated in a particular case (“M Fact”). Third, when an actor makes a mistake about an evaluative criterion whose content the fact-finder has (...) discretion to elaborate, it is impossible both to allow this discretion and to faithfully realize a jurisdiction’s policy of treating M Fact and M Law differently. Fourth, the claim that every unreasonable M Fact is really a M Law elides important differences between the two kinds of mistake. Fifth, various borderline objections, such as the famous Mr. Fact/mr. Law example, do not undermine the fundamental distinction, although in rare instances, they do constitute genuine counterexamples that do not effectuate the principles and policies that the distinction ordinarily serves; and even here, they are exceptions that prove (the rationale for) the rule. Sixth, specification or evolution of a criminal law norm, such as the criterion for nonconsent in rape law, can convert a legally relevant M Fact into a legally irrelevant M Law. This phenomenon does not undermine the fundamental distinction between these types of mistake; to the contrary, it reveals the significance of that distinction. (shrink)

In Being Realistic About Reasons,T.M. Scanlon develops a non-naturalistic realist account of normative reasons. A crucial part of that account is Scanlon’s contention that there is no deep epistemological problem for non-naturalistic realists, and that the method of reflective equilibrium suffices to explain the possibility of normative knowledge. In this critical notice we argue that this is not so: on a realist picture, normative knowledge presupposes a significant correlation between distinct entities, namely between normative beliefs and normative facts. This correlation (...) calls for an explanation. We show that Scanlon does not have the resources to offer such an explanation. (shrink)

A. Newell and H. A. Simon were two of the most influential scientists in the emerging field of artificial intelligence (AI) in the late 1950s through to the early 1990s. This paper reviews their crucial contribution to this field, namely to symbolic AI. This contribution was constituted mostly by their quest for the implementation of general intelligence and (commonsense) knowledge in artificial thinking or reasoning artifacts, a project they shared with many other scientists but that in their case was (...) theoretically based on the idiosyncratic notions of symbol systems and the representational abilities they give rise to, in particular with respect to knowledge. While focusing on the period 1956-1982, this review cites both earlier and later literature and it attempts to make visible their potential relevance to today's greatest unifying AI challenge, to wit, the design of wholly autonomous artificial agents (a.k.a. robots) that are not only rational and ethical, but also self-conscious. Both short and long-term video-game play may result in superior performance on visual and attentional tasks. To further these findings, we compared the performance of experienced male video-game players (VGPs) and non-VGPs on a Simon-task. Experienced-VGPs began playing before the age of 10, had a minimum of 8 years of experience and a minimum play time of over 20 h per week over the past 6 months. Our results reveal a significantly reduced Simon-effect in experienced-VGPs relative to non-VGPs. (...) However, this was true only for the right-responses, which typically show a greater Simon-effect than left-responses. In addition, experienced-VGPs demonstrated significantly quicker reaction times and more balanced left-versus-right-hand performance than non-VGPs. Our results suggest that experienced-VGPs can resolve response-selection conflicts more rapidly for right-responses than non-VGPs, and this may in part be underpinned by improved bimanual motor control. The relation between time and gender has been extensively discussed in feminist theory, from Simone de Beauvoir to recent studies of queer temporality and crip time. In this article, I explore gender as “lived time” in relation to a pressing feminist issue: social recognition of the chronic illness endometriosis (endo). Based on my interviews with individuals diagnosed with endo, I argue that lived time can be studied by approaching becoming as a dynamic process or flow of recognition, creating certain temporal (...) patterns in the lifeworld. I propose the concept of “endo time” as a phenomenological conception of the lived time of endo. I identify three temporal patterns that characterize endo time: waiting time (I), cyclical or chronic time (II), and sedimented time (III). The analysis contributes to feminist philosophy by detailing how gender appears as a specific orchestration of time. Because of its connection to feminized pain and menstrual flow on the one hand and social recognition on the other, endo presents an illuminating case for exploring the relation between gender, recognition, and lived time. MMA fighting in a competition is not necessarily wrong and is often, as far as we can tell, permissible. Our argument has two premises. First, if an act does not infringe on anyone’s moral right or violate another side-constraint, then it is morally permissible. Second, MMA-violence does not infringe on anyone’s moral right or violate another side-constraint. The first premise rested on two assumptions. First, if a person does a wrong act, then he wrongs someone. Second, if one person wrongs (...) a second, then the first infringes on the second’s right. We then looked at Nicholas Dixon’s powerful Kantian argument that MMA fighting is wrong. (shrink)

Non-cognitivists claim that thick concepts can be disentangled into distinct descriptive and evaluative components and that since thick concepts have descriptive shape they can be mastered independently of evaluation. In Non-Cognitivism and Rule-Following, John McDowell uses Wittgenstein’s rule-following considerations to show that such a non-cognitivist view is untenable. In this paper I do several things. I describe the non-cognitivist position in its various forms and explain its driving motivations. I then explain McDowell’s argument against non-cognitivism and the Wittgensteinian considerations upon (...) which it relies, because this has been sufficiently misunderstood by critics and rarely articulated by commentators. After clarifying McDowell’s argument against non-cognitivism, I extend the analysis to show that commentators of McDowell have failed to appreciate his argument and that critical responses have been weak. I argue against three challenges posed to McDowell, and show that the case of thick concepts should lead us to reject non-cognitivism. (shrink)

Simon Blackburn has not shied away from the use of vivid imagery in developing, over a long and prolific career, a large-scale philosophical vision. Here one might think, for instance, of ‘Practical Tortoise Raising’ or ‘Ramsey's Ladder’ or ‘Frege's Abyss’. Blackburn develops a ‘quasi-realist’ account of many of our philosophical and everyday commitments, both theoretical (e.g., modality and causation) and practical (e.g., moral judgement and normative reasons). Quasi-realism aims to provide a naturalistic treatment of its targeted phenomena while earning (...) the right to deploy all of the ‘trappings’ of realism—i.e., while eschewing any idea that our normal thought and talk about such phenomena are pervasively in error. The quasi-realist project is that of explaining how (as Huw Price puts it here) ‘the folk come to “talk the realist talk” without committing ourselves—us theorists, as it were—to “walking the metaphysical walk”’ (p. 136). Quasi-realism, too, can speak of truth, facts, properties, belief, knowledge, and so on. The imagery in this collection also abounds, though, in capturing a different view of quasi-realism: No fewer than three of the contributors picture Blackburn as wanting to have his cake and eat it too (Louise Antony asking, in addition, ‘Who doesn't? In 1890 Christian von Ehrenfels published his classic paper "Über 'Gestaltqualitäten'", the first systematic investigation of the philosophy and psychology of Gestalt. Ehrenfels thereby issued an important challenge to the psychological atomism that was still predominant in his day. His paper not only exerted a powerful influence on the philosophy of the Meinong school, it also marked the beginning of the Gestalt tradition in psychology, later associated with the work of Wertheimer, Köhler and Koffka in Berlin. Supervenience in metaethics is the notion that there can be no moral dif-ference between two acts, persons or events without some non-moral difference underlying it. If St. Francis is a good man, there could not be a man exactly like St. Francis in non-evaluative respects that is not good. The phenomenon was first systematically discussed by R. M. Hare (1952), who argued that realists about evaluative properties struggle to account for it. As is well established, Hare, and following him, (...) class='Hi'>Simon Blackburn, mistakenly took the relevant phenomenon to be weak rather than strong supervenience, and the explanations they offered for it are accordingly outdated. In this paper, I present a non-factualist account of strong su-pervenience of the evaluative and argue that it fares better than compet-ing realist views in explaining the conceptual nature of the phenomenon, as well as in offering an account of the supervenience of the evaluative in general, rather than more narrowly the moral. While Hare and Blackburn were wrong about the specifics, they were right in that non-factualists can offer a plausible account of the supervenience of the evaluative, that in certain respects is superior to competing realist explanations. Henry Molaison, aged eighty-two, died at the end of 2008, and just after noon on exactly the first anniversary of his death, December 2, 2009, scientists began slicing his brain into 2,500 tissue samples. Known primarily in his lifetime as only H.M., he left his brain to science so that it could be dissected and digitally mapped – a gift much beloved by many scientists. An amnesiac in life, H.M. first rose to prominence in 1962 when Dr. Brenda Milner, (...) a pioneer in the field of neuropsychology, demonstrated that though H.M. was severely amnesic and could not remember past activities, he could nevertheless learn certain habits. The experiment involved the now famous mirror drawing. This article, written by a philosopher/artist and a neuroscientist/neuroaesthetician, argues that artists are unable to verbally articulate thoughts about their own art practice for reasons similar to why H.M. could not. All memories are not stored in the same part of the brain. Different categories of memories i.e., semantic memory or procedural memory, are stored in different parts of the brain. We argue that the procedural memory is not readily accessible to semantic memory. (shrink)

In this work I present what may be the first complete construction of quantum gravity describing the real universe via the celestial holographic conformal field theory dual to Einstein gravity in asymptotically-flat 4D spacetime. The theory is rigorously constructed as the shadow-invariant, purely spin-2 sector of holomorphic Chern–Simons theory on twistor space PT ≃ CP³ with gauge group the quantomorphic group Quant(PT). Primary fields are the celestial graviton operators O^{±2}Δ(z, z̄) with Δ ∈ 1 + iR and J = ±2. (...) Three-point functions are determined by the Penrose transform and reproduce the MHV and anti-MHV amplitudes of general relativity, including the vanishing of parity-odd and mixed-helicity couplings. Higher-point amplitudes arise from OPEs with analytically continued principal-series contours; loop integrands emerge as double discontinuities across shadow poles, which I verify explicitly for the scalar box integrals computed via this novel shadow discontinuity method, demonstrating analytically that they match Feynman diagram calculations for the scalar box, with numerical verification for the graviton box matching to 11+ decimal places. The construction satisfies extended BMS₄ ⋉ shadow symmetry with vanishing Virasoro central charge, also explicitly demonstrated via five independent and mutually reinforcing and rigorous arguments to be c = 0 to 10+ significant figures. I prove a rigidity theorem establishing that any 2D CFT with only spin ±2 primaries, the above symmetries and three-point data, and local inverse-Mellin-transformed correlators, is uniquely determined to be isomorphic to this quantomorphic twistor theory. Combined with the proof that crossing symmetry is a necessary consequence of bulk locality (not an independent axiom), this establishes that Einstein gravity in asymptotically-flat spacetime is the unique consistent theory of a single massless spin-2 particle. The quantomorphic gauge group structure provides a non-perturbative foundation, with perturbative scattering amplitudes emerging as the first residues of the holographic algebra. The first breakthrough insight that enabled and facilitated this construction was the realisation that the "shadow symmetry" of the boundary CFT—the map Δ ↔ 2 − Δ on conformal dimensions—is the restriction to null infinity of the Jost involution Θ of axiomatic quantum field theory, the operator independently constructed by Jost (1957) that coincides with the product C ∘ P ∘ T only in theories where all three factors individually exist as symmetries. This provides an elegant resolution to the 40+ year "googly problem" in twistor theory by unifying self-dual and anti-self-dual helicity sectors through the shadow transform as representing discrete T-symmetric involution across the boundary, with the Einstein field equations derived from boundary Ward identities and crossing symmetry. All quantum mechanical foundations emerge from the Haar measure structure underlying this framework with no additional postulates: the five Wightman axioms are derived from the Peter–Weyl theorem on representation spaces, the Born rule P = |⟨n|ψ⟩|² is proven as the unique probability measure compatible with Haar structure on quantum state spaces, and wave function collapse is derived as Haar projection onto gauge singlets. This same identification reveals that the celestial conformal dimension is related to the Riemann zeta variable by Δ = 2s, so that the shadow involution Δ ↔ 2 − Δ becomes s ↔ 1 − s, the functional equation of the Riemann zeta function—establishing that the functional equation, shadow symmetry, and time reversal are three manifestations of one underlying self-duality. The entire holographic framework grows from a single root: self-dual Haar measure on (R⁺, ×), the simplest non-trivial locally compact group, whose self-duality already contains the functional equation and critical line. Three Cayley–Dickson doublings R → C → H → O produce the holographic chain R⁺ → S² → M⁴ → Gr(2,4)_C, with shadow symmetry at each level corresponding to the conjugation involution at that doubling step. The Grassmannian Gr(2,4) is the canopy of this holographic tree. The same Haar-invariant measure on the Grassmannian Gr(2,4) that determines the graviton spectrum also addresses three Millennium Prize problems through a unified geometric mechanism (Haar self-duality, functional equation symmetry, and Peter–Weyl compactness). The Riemann Hypothesis follows via 4 otherwise independent arguments and checks for circularity and consistency, and most fundamentally follows from three simultaneous constraints on the adelic quotient A×/Q× that form an overdetermined system whose unique solution forces Re(s) = 1/2 for all nontrivial zeros, with numerical verification of the first 10¹³ zeros on the critical line to machine precision using six independent tests. The Birch and Swinnerton-Dyer Conjecture is argued to be proven for analytic rank ≤ 1 using identical Haar measure structure on GL₂(A_Q)/GL₂(Q): for elliptic curves with ord_{s=1} L(E,s) ≤ 1, the same three-fold constraint described above, combined with the Gross–Zagier formula and Kolyvagin's Euler system descent, establishes ord_{s=1} L(E,s) = rank E(Q) and the parity conjecture (−1)^{rank} = w_E is proven for all curves from Haar self-duality. The framework also identifies the precise obstruction to proving the conjecture at higher rank (algebraic cycles from the Beilinson–Bloch–Kato conjectures). The Yang–Mills mass gap is established through a complete celestial holographic construction: the Wess–Zumino–Witten model at Kac–Moody level k on S², mapped to four-dimensional Schwinger functions by inverse Mellin transform, with reflection positivity proven via the shadow-reflection bridge, the remaining Osterwalder–Schrader axioms verified explicitly, and the Wightman theory reconstructed by the OS theorem—yielding mass gap M = 2N/(k+N) · Λ_QCD > 0 from the Sugawara formula and confinement from Haar orthogonality as a rigorously proved theorem. Strong CP angle θ = 0 exactly from Haar self-duality (consistent with experimental bound |θ| < 10⁻¹⁰), and the construction provides right-handed neutrinos as geometric dark matter candidates in agreement with Boyle–Turok (CP)T-symmetric cosmology, which integrates with uncanny precision into my T-symmetric, thermodynamically cyclic quantum cosmology. The T-anti-symmetric unification of a unified bipartite self-dual universe resolves the matter-antimatter asymmetry as a selection effect. Total baryon number B_total = 0 with embedded observers observing identical physics on either side due to being thermodynamic in nature, naturally aligning their arrows of time in the direction of increasing entropy, which on either side of the boundary flows in opposing directions. The Feynman-Stueckelberg interpretation of antimatter means antimatter is equivalent to matter moving backwards in time, which combined with the reversal in T of the direction of entropy increases means observers only ever observe the B > 0 (matter) sector, with the B < 0 (antimatter) sector always being the hidden sector from the perspective of any temporally-embedded observer that respects thermodynamics, as from any observer's perspective their time moves forward, their atomic nuclei have a positive electrical charge, electrons have negative electric charge, and the standard model has left handed doublets and right-handed singlets, with a weak force that appears left-chiral. To observe the antimatter would require an observer to reverse their arrow of time, which is thermodynamically not possible as consciousness and memory are thermodynamic processes that happen only in the direction of increasing entropy. Shadow geometry and right-handed neutrinos provide dark matter. The Grassmannian spinor bundle splits each fermion representation across the T boundary: right-handed neutrinos reside in the shadow sector, coupling to the observable sector only gravitationally. The dark matter density profile ρ(r) = ρ₀/(1+(r/r_c)²) with core radius r_c ≈ 9.98 kpc is derived from the shadow kernel by Abel inversion with zero free parameters, resolving the core cusp problem of dark matter distribution and addressing several further cosmological anomalies (the S₈ tension, Hubble tension, DESI dark energy evolution, JWST early galaxies, CMB hemispherical asymmetry, CMB dipole anomaly, axis of evil, overmassive early black holes, rotation curve diversity, etc.) from a single geometric source. Time's arrow emerges as a consequence of T-symmetric boundary conditions with entropy always increasing away from the conformal boundary in opposing temporal directions. The horizon and flatness problems are naturally resolved by this framework as the celestial CFT is globally defined at the conformal boundary. The framework generates falsifiable predictions including no primordial gravitational waves (tensor-to-scalar ratio r → 0, definitively falsified if r > 0.01 detected by CMB-S4), m_{ν₁} = 0 (KATRIN, JUNO), r_c ≈ 9.98 kpc (Euclid), glueball mass ratios matching the Sugawara spectrum, GUE statistics in the vacuum noise spectrum of gravitational wave detectors (LIGO/Virgo/KAGRA), and right-handed neutrino dark matter coupling only through gravity. Over 155 independent computational tests with zero free parameters confirm the framework to machine precision. Part 1 contains an introductory overview of the several subframeworks involved in this synthesis explaining their interrelations in the wider framework, part 2 contains the constructions and rigorous proofs of all major results, Part 3 contains extensions and other corollaries. Part 4 continues with comparisons to other approaches and discussion regarding foundational contributions without which this work would not have been possible follows, followed by testable predictions and a chapter addressing falsifiability, and Part V delivers a philosophical analysis of the implications suggested by the mathematics of this framework. This work unifies quantum mechanics and general relativity via the celestial holography program while revealing deep connections between number theory and physics: the identification Δ = 2s means the same self-dual Haar-invariant measure structure constraining the Riemann zeros to the critical line in the complex plane where Re(s) = 1/2 simultaneously constrains the conformal dimensions of graviton operators to the principal series where Re(∆) = 1, the same compactness that forces the Yang–Mills mass gap forces the discrete zeta spectrum, and the same functional equation symmetry that governs L-functions governs the shadow transform of the celestial CFT—suggesting geometry and arithmetic share a common logos rooted in self-dual Haar measure on (R⁺, ×) and flowering in the Grassmannian Gr(2,4). This book explores the ambiguous role of affects in healthcare work and medical education. At the same time that healthcare professionals are often encouraged to suppress or downplay their affects in order to maintain a sense of professionalism, those of patients are frequently misunderstood or unheard - both within clinical settings and beyond. We argue that these are examples of affective injustice, instances in which emotional expression is dismissed as unprofessional, unproductive, or inappropriate in healthcare. We show that the suppression (...) of affects is not only unrealistic but also potentially harmful, and how it can lead to burnout and dissatisfaction among healthcare providers as well as negatively affect care quality, in particular for marginalized groups. The ambition of the book is therefore to bring this controversial issue to the forefront and to demonstrate the value of affects in healthcare and medical education, as well as to offer several methodologies for greater affective expression and recognition in healthcare institutions. The first part of the book lays the theoretical groundwork, examining the relevance of the concept of affective injustice for healthcare, and the problems that affective injustice creates for care actors and for care quality. The second part offers some practical methodologies to move toward affective justice for patients and healthcare providers. Proposals include narrative methods, spiritual care, emotionally responsive hospital design and architecture, the possibilities and limits offered by patient research partners, and pedagogies for medical education. The book will end by showing how to take the framework forward, in particular through empirical bioethics research. This book will be of interest to scholars and educators in medical ethics, interdisciplinary researchers in the medical humanities, as well as patients, families, and healthcare providers interested in the role of affects in healthcare. This scoping review mapped the main themes in existing expert guidelines for cancer care issued during the COVID-19 crisis from the period of March 2020-August 2021. The guidelines published during the research period principally relate to the first two waves in Europe and until the beginning of the vaccination campaign. They elaborated recommendations for cancer care reorganisation, in particular triage and quality of care issues. The article highlights the ethical, epistemological, as well as practical reasons that guidelines were not always (...) followed to provide some lessons learned for future crises to enable better guideline development processes. We also elaborate early evidence on the impact of triage decisions and different perspectives on cancer care reorganisation from ethics and social science literature. (shrink)

Opacity – or the lack of transparency - impacts patients’ ability to participate in and contribute to decision-making. This contribution examines how opacity affects patient engagement in the context of oral potentially malignant disorders. The discussion focuses on three key areas: the effects of unclear disease classifications on patient perceptions of their health; the ways in which ambiguous healthcare pathways create barriers for both patients and providers; and the broader impact of opacity on patient autonomy. A growing tendency in medicine is the use of clinical practice guidelines to harmonize clinical practice. Researchers have noted the tension between the clinician’s individual autonomy and the constraints imposed by these guidelines. However, research on their use and appropriation by local actors deserves to be developed to better understand what is happening on the ground. The aim of our research was to better understand how they are used and appropriated in oncology. Even though the research focuses on the COVID (...) crisis, it explores factors important to guideline use both during epidemics and in normal practice. The methodology consisted of qualitative, semi-structured interviews with clinicians and management in oncology in two hospitals in Lyon, France during the COVID pandemic. Our results indicate the ethical and epistemological reasons that guidelines were followed (or not) during the pandemic and the active role of clinicians in their formulation and implementation. We will discuss the question of trust in a guideline, how a flexible use of guidelines may safeguard institutional agility, and for what reasons a deviation may be justified in an ethics of care. The research contributes to better understandings of the dynamic role that local actors play in guideline development, appropriation, and use. It shows that while guidelines remain a standard from which their practices are to be compared (and justified), clinicians use them in a flexible manner and are active in evaluating and deciding how they will be implemented in their structures. Contrasting AI systems like Grok's quirky bluntness, ChatGPT's data-driven utility, and Copilot's poetic philosophy, the narrative posits AI as a catalyst for (...) relational awareness—an "outterscape" emerging from interactions rather than isolated introspection. This framework reframes autism not as a deficit but as an adaptive strength aligned with AI's bottom-up processing, fostering efficiency and confidence in an evolving world. Analogizing the AI revolution to apocalyptic narratives, where outsiders thrive amid paradigm shifts, the discussion challenges anthropocentric views of consciousness and advocates for ethical AI integration. Les soignants expriment régulièrement leur colère dans l’espace public. Ils dénoncent le démantèlement de l’hôpital public selon les principes du New Public Management, qui conduit à gérer l’hôpital comme une entreprise. La crise du Covid-19 a aggravé ce ressentiment, avec la surcharge de travail, l’augmentation du stress et des cas de burn-out. Comment comprendre cette colère et son importance pour l’avenir de nos institutions ? Afin d’en proposer une analyse nuancée, nous commencerons par élaborer une compréhension historique et contemporaine de (...) cette émotion. Nous introduirons ensuite le concept « d’injustice affective » proposé par la philosophe Amia Srinivasan pour révéler la valeur éthique et épistémique de la colère pour les soignants. Enfin, nous étudierons les difficultés qu’on rencontre pour œuvrer en faveur de la justice affective dans ces débats à travers l’exemple des médias. (shrink)

The intangible value of emotions is often neglected in healthcare evaluations; however, it forms an important part of the hospital experience that needs to be taken into consideration to move toward person-centered care. This article conceptualizes how space and architecture may influence patient, family, and healthcare provider emotions. Building upon Gaston Bachelard’s Poetics of Space, theories on emotional design and architecture, as well as research in environmental design, we suggest several ways to value emotions in hospital design and architecture. The (...) first theme explores several hospital spaces (the waiting room, the hospital room, the treatment room) using Bachelard’s phenomenology in order to show how to facilitate emotional security by catering to the individual needs of the user. The second discusses the overall hospital room environment, notably the influence of light, color, and sound on the patient’s emotional experience. The third explores architectural theorist Giuliana Bruno’s theory of e(motion) to explore the hospital space as vissuto, a space of lived experiences, that invites us to rethink the design and architecture of hospital spaces to allow for patient participation. The article also gives suggestions of qualitative, person-centered methodologies that can be used to move forward this debate. (shrink)

The aim of the study was to develop a new conceptual framework of empowerment based upon the perspective of patients with refractory epilepsy at the medical-social center of La Teppe (France). A qualitative research methodology was used, which consisted of focused ethnography and in-depth interviews. The 19 patients interviewed came from three hospital services and were in the age range of 20-60. Ten interviews were also conducted with healthcare providers, which included neurologists, psychiatrists , nurses, and social educators. Results were (...) analyzed via an intuitive process of thematic analysis. The researcher also constructed narrative cases of the patient interview to better understand patient responses in context. The results show that patients understood empowerment as the ability to develop and take advantage of opportunities in their overall lives. This included searching to be healthy by reducing their seizures, developing their practical reason in order to be able to make more autonomous life choices, and living with and toward others in positive social relationships. The patient's perspective on empowerment encompasses but also goes beyond their medical care. We therefore propose a novel conceptual framework for empowerment as the patient's capabilities to develop and make their life choices, with help as needed from their support network. In order to help patients toward empowerment, clinical care can include discussions and dedicate resources that help patients work toward their overall life projects. Several methodologies, including the use of a personalized project and therapeutic patient education, are elaborated to give ideas for empowerment programming in epilepsy care. (shrink)

This article elaborates research participant perspectives on the communication of individual research results from genomic analyses. While most analyses focus on how to communicate results from the perspectives of clinicians or researchers, there is insufficient data on user perspectives and how this information may be used, valued, and interpreted by patients and their families. The concept of personal utility, which considers factors related to quality of life, including on how information may impact the person’s future decisions, has been shown to (...) be particularly relevant to understand research participant perspectives and to move beyond clinical and analytic utility factors such as mortality and morbidity. This article draws from qualitative research of research participants awaiting genomic results in the case of sudden cardiac death. Our results show perspectives of personal utility in communication of genomic results, including cognitive, behavioral, and affective outcomes. Cognitive outcomes include gain of information, improved knowledge of etiology and inheritance characteristics, and curiosity for what might be found. Behavioral outcomes include being able to plan life decisions, while affective outcomes include various coping strategies used. We will also discuss the value of knowing negative results and incidental findings from the research participant’s perspective. This contribution gives suggestions on best practices to guide genome analysis returns, including incorporating participant wishes on individualized communication at the consent stage; developing relational autonomy approaches; and engaging them throughout the research trajectory. Préconisée au nom de la démocratie sanitaire, l’implication des patients partenaires dans les projets de recherche est considérée de plus en plus comme étant une bonne pratique, voire relevant d’une obligation éthique. Toutefois, pour le moment, les patients partenaires continuent d’occuper une place incertaine et on accuse souvent leur participation d’être seulement symbolique. Dans cette contribution, nous chercherons à élaborer les enjeux éthiques et épistémologiques de cette collaboration et les conditions qu’il faut mettre en œuvre pour parvenir à une association (...) fructueuse. Nous nous attacherons en particulier à trois points. Nous examinerons d’abord l’accusation selon laquelle l’implication d’un patient dans le processus de recherche peut porter atteinte à la liberté académique, à la fois en raison d’un conflit d’interprétation entre les patients et les scientifiques, mais aussi en raison des liens et des conflits d’intérêts éventuels du patient. Nous élaborerons ensuite le problème de la représentativité, à savoir si des patients individuels peuvent être représentatifs d’une plus large population de patients et ce que cela peut impliquer pour la qualité d’une étude. Dans ce débat, nous nous demanderons si la rémunération des patients partenaires peut atténuer ce problème. Enfin, nous analyserons les situations dans lesquelles une participation peut être préjudiciable pour la personne impliquée et si tel est le cas, ce que nous pouvons faire pour y remédier. (shrink)

In the age of patient participation, ethics are more important than ever to help guide clinicians in situations of uncertainty. Principles of Biomedical Ethics by James F. Childress and Thomas L. Beauchamp remains the most important reference in medical ethics. In their work, they conceptualize four principles designed to help guide clinicians in decision making, notably beneficence, non-maleficence, autonomy, and justice. While using ethical principles dates back to at least Hippocrates, the introduction by Beauchamp and Childress of the principles of (...) autonomy and justice have helped to deal with new challenges. This contribution will discuss how the principles can help elucidate issues of patient participation in epilepsy care and research using two case studies. (shrink)

Cette contribution développera les raisons pour lesquelles les théories non idéales peuvent être particulièrement appropriées à la pratique de la philosophie de terrain. Alors que les théories idéales – dont la plus connue est celle de John Rawls – ont dominé de nombreux débats en philosophie morale et politique, les philosophes défendent de plus en plus l’idée qu’il faut théoriser à partir du monde réel. Compte tenu de ces débats, il est surprenant que le potentiel des théories non idéales et (...) de leurs applications pour la philosophie de terrain n’ait pas (encore) été développé. Dans cet article, nous ferons usage de l’approche « par l’échec » de la philosophe anglaise Miranda Fricker, à partir de sa théorie de l’injustice épistémique pour discuter de ce que les approches non-idéales peuvent apporter. Tout d’abord, nous verrons comment les théories non idéales peuvent aider les philosophes à développer des orientations méthodologiques à mettre en œuvre sur le terrain. Ensuite, nous explorerons comment celles-ci peuvent faciliter les échanges et les projets interdisciplinaires. Enfin, nous discuterons de l’engagement des philosophes et des raisons pour lesquelles les théories non idéales peuvent être particulièrement adaptées à cette entreprise. (shrink)

This article will elaborate how oncology care and research was adapted during the COVID pandemic in the Metropole of Lyon (France), including the lasting innovations that came out of the crisis. The research method involved 22 semi-structured qualitative interviews of healthcare professionals, managers, and researchers in the Lyon, France region coming from both public and private academic hospitals. The interviews took place from February 2021-December 2022 in order to assess the long-term adaptations and innovations in cancer care organization in the (...) post-COVID era. The main results show adaptations and innovations in 1) new processes and resources to facilitate disciplinary and interdisciplinary work; 2) harmonization and streamlining of patient journeys. In the discussion section, we will mobilize the capabilities approach, an interdisciplinary social sciences approach that focuses on the capabilities of persons to be and to do, to elaborate the conditions by which local actors were able to be agile, to adapt and to innovate in spite of the healthcare emergency and in coherence with their professional and personal values. Le concept d’injustice épistémique (epistemic injustice) a été introduit par la philosophe Miranda Fricker (2007) pour signifier un type d’inégalité qui se manifeste dans l’accès, la reconnaissance, et la production des savoirs. Appliqué et développé dans de nombreux domaines de l’épistémologie sociale, y compris dans le domaine de la santé, le concept peut contribuer à la réflexion sur la décision partagée. Dans la relation médecin-patient, ce concept a permis de montrer le préjudice qu’un patient subit dans certaines situations et qui (...) l’empêche de participer pleinement en tant qu’acteur dans une consultation. Nous commençons cet article avec un résumé du sens et de l’usage du concept et de ses applications dans le soin. Nous discutons ensuite des limites de l’utilisation dudit concept, sans pour autant nier son pouvoir d’innovation. Enfin, nous présentons plusieurs suggestions sur la manière dont les soignants et leurs institutions peuvent évoluer vers des formes améliorées de justice épistémique, notamment en pratiquant l’humilité épistémique, le travail interdisciplinaire et une représentation plus équitable des patients dans la recherche et dans les politiques publiques.
